ABSTRACT

Objective To evaluate the effect of information-knowledge-attitude-practice (IKAP) theory-based health education interventions on clonorchiasis control among community residents and primary school students in Torch High-tech Development Zone of Zhongshan City, so as to provide insights into formulation of clonorchiasis control strategies among different types of populations. Methods Residents were randomly sampled from two communities and students from two primary schools using a random cluster sampling method in Torch High-tech Development Zone, Zhongshan City from 2019 to 2021, and health education pertaining to clonorchiasis control was performed based on the IKAP theory. The changes in the awareness of basic clonorchiasis control knowledge and belief, self-reported risk of Clonorchis sinensis infections and dietary behaviors were compared among community residents and primary school students before and after health education interventions. Results The participants included 146 male and 151 female community residents and 158 male and 153 female primary school students, with no significant difference detected in gender distribution (χ2 = 0.16, P > 0.05). The mean awareness of basic clonorchiasis control knowledge increased by 44.71% and 43.28% among primary school students and community residents 6 months following health education, and there were significant differences in the awareness of each item of basic clonorchiasis control knowledge before and after health education (χ2 = 41.53 to 284.44, all P values < 0.05). The proportions of primary school students and community residents thinking very high and high risks of C. sinensis infections increased from 9.35% and 6.71% before health education to 22.15% and 37.75% after health education, but only the difference of the attitudes in community residents achieve statistical significance (χ2 = 92.18, P < 0.05). The frequency of separation of raw and cooked foods with chopping board and knife significantly increased among community residents and primary school students following health education (χ2 = 16.04 to 62.65, all P values < 0.05). The frequency of eating raw freshwater fish (χ2 = 32.85, P < 0.05), fish congee (χ2 = 7.02, P < 0.05) and fish fillet hot pot (χ2 = 4.88, P < 0.05) significantly reduced among primary school students following health education, while only the frequency and proportions of eating raw freshwater fish have significantly reduced in community residents (χ2 = 11.77, P < 0.05). Conclusions Health education interventions based on the IKAP theory are effective to increase the awareness of clonorchiasis control knowledge and improve dietary behaviors associated with C. sinensis infections among community residents and primary school students in Zhongshan City.

ABSTRACT

Objective@#To investigate the prevalence of anemia and its influencing factors among primary school students from Kazakh ethnic groups in agricultural and pastoral areas of Altay Prefecture, Xinjiang Uygur Autonomous Region, so as to provide insights into improvements of nutrition and prevention of anemia among students.@*Methods@#Grade 1 to 3 Kazakh ethnic students were sampled using a cluster sampling method from 7 primary schools in agricultural and pastoral areas of Altay Prefecture, Xinjiang Uygur Autonomous Region from April to June, 2022. Demographics and dietary behaviors were collected using questionnaire surveys, and the height and body weight were measured, while peripheral blood hemoglobin was detected. Factors affecting anemia were identified using a multivariable logistic regression model.@*Results@#A total of 2 378 questionnaires were allocated, and 2 214 valid questionnaires were recovered, with an effective rate of 93.10%. The respondents included 1 123 men (50.72%) and 1 091 women (49.28%), and had a mean age of (8.12±1.41) years. The prevalence of anemia was 22.49%, and the detection rates of mild and moderate anemia were 15.31% and 7.18%, with no severe anemia detected. Multivariable logistic regression analysis identified frequency of eating eggs (0, OR=2.532, 95%CI: 2.114-3.015; 1 to 3 times/week, OR=1.926, 95%CI: 1.648-2.216), frequency of eating meat (1 to 3 times/week, OR=1.468, 95%CI: 1.256-1.689; 4 to 7 times/week, OR=2.212, 95%CI: 1.823-2.641), frequency of healthy check-up (0, OR=2.872, 95%CI: 2.523-3.231; 1 time/year, OR=1.826, 95%CI: 1.546-2.108), monthly mean household income (1 500 Yuan and lower per month, OR=1.324, 95%CI: 1.071-1.582; 1 501 to 3 000 Yuan per month, OR=1.124, 95%CI: 1.029-1.221) and caregiver (grandparents, OR=1.175, 95%CI: 1.038-1.324; others, OR=1.241, 95%CI: 1.032-1.466) as factors affecting anemia.@*Conclusions@#The prevalence of anemia is 22.49% among primary school students from Kazakh ethnic groups in agricultural and pastoral areas of Altay Prefecture, Xinjiang Uygur Autonomous Region, with mild anemia as the predominant type. Dietary behaviors, frequency of healthy check-up, economic status and caregiver may affect the development of anemia.

ABSTRACT

Objective@#To evaluate the caries preventive effect and economic outcome of an oral health promotion model in rural primary schools in Guangxi Zhuang Autonomous Region.@*Methods@# In November 2015, a baseline survey was conducted on the caries prevalence of rural first graders in program areas, and then a program intervention was carried out on the first graders of target primary schools. The intervention methods included the distribution of oral health care products, oral health education, teacher training, etc. In 2020, the method of cluster random sampling was adopted to include the sixth graders who received the program intervention as first graders and were still available in 2020 as the intervention group. During this period, the sixth graders transferred from foreign schools who failed to receive the program intervention as first graders were included in the non-intervention group. The caries preventive effect was evaluated by analyzing the caries epidemiological data of 2 652 children aged 11-13 years, comparing the intervention and non-intervention group. The cost of the program was estimated by the structural analysis method. A decision tree model was established by TreeAge pro2019 and incorporated into the effect parameters and cost parameters for cost-effectiveness analysis and sensitivity analysis.@* Results @# The prevalence of caries in the intervention group was 54.8%, and the mean DMFT was 1.36 ± 1.64, both of which were lower than those in the non-intervention group. The difference in prevalence and mean DMFT between the two groups was statistically significant (P < 0.05). It costs CNY 319.83 per child to reduce suffering from caries. The number of patients with caries in the intervention group was the most sensitive indicator of an economic effect. The probability of a cost-effectiveness advantage for the program was 92.2%. @*Conclusion @# This oral health promotion model used in rural schools demonstrated a caries preventive effect that was very likely economically advantageous. It is of practical significance to improve and promote this model in the Guangxi Zhuang Autonomous Region.

ABSTRACT

Objective@#To a evaluate the effectiveness of comprehensive nutrition interventions among primary school students in Yiwu City, so as to provide insights into malnutrition control among children.@*Methods@#Grade 3 to 5 students were sampled from four primary schools in Yiwu City using a random cluster sampling method and randomly assigned into the intervention group and the control group. Students in the intervention group received comprehensive interventions, including nutritional health education, promotion of physical activities, nutritional meal support and creation of a nutritional campus, while students in the control group were given no interventions. The awareness of nutritional health knowledge, dietary behaviors and nutritional status were compared in students between the two groups prior to interventions and one year following interventions, and the effectiveness of interventions was evaluated using generalized estimating equations.@*Results@#Totally 879 students were enrolled. There were 440 students in the intervention group, including 243 males and 197 females, with a male to female ratio of 1︰0.81 and a mean age of (10.47±0.99) years; and there were 439 students in the control group, including 244 males and 195 females, with a male to female ratio of 1︰0.80 and a mean age of (10.35±1.02) years. Following comprehensive interventions, the awareness of “type of food”, “seven nutriments from food”, “eating at least 12 types of food daily”, “less than 6 g of daily salt intake”, “food composition in nutritional breakfast”, “nutritional labels of pure milk”, “no less than 60 min of daily exercise duration” and “too fat or too thin may threaten health” and the increase in the proportion of 3 and more types of food in breakfast were significantly higher among students in the intervention group than in the control group (all P<0.05); however, there was no statistical difference in the proportion of normal nutrition between the two groups (P>0.05).@*Conclusion@#The comprehensive nutritional interventions may effectively increase the awareness of nutrition health knowledge and improve dietary behaviors among primary school students.

ABSTRACT

Objective@#To investigate the awareness of adolescent sexual knowledge among primary school students in Hangzhou City, so as to provide insights into adolescent sex education.@*Methods@#In May 2021, grades 5 and 6 students were sampled from 5 primary schools in urban areas and 5 primary schools in suburban areas of Hangzhou City. The demographic features, awareness of adolescent sexual knowledge and sources of acquiring adolescent sexual knowledge were investigated using electronic questionnaires, and the awareness of adolescent sexual knowledge were analyzed among primary school students.@*Results@#Totally 1 812 students completed valid questionnaires, including 861 grade 5 students ( 47.52% ) and 951 grade 6 students ( 52.48% ), 936 boys ( 53.86% ) and 836 girls (46.14%), and 912 students living in urban areas (50.33%) and 900 students living in suburban areas ( 49.67% ). The awareness rates of adolescent physiological knowledge, adolescent health knowledge, private parts and sexual harassment behaviors were 53.20%, 46.30%, 60.49% and 92.55%, respectively, and the awareness rates of adolescent physiological knowledge ( 69.52% vs. 39.14%; χ2=168.001), adolescent health knowledge (55.74% vs. 38.22%; χ2=55.619 ), private parts ( 72.49% vs. 50.20%; χ2=93.550 ) and sexual harassment behaviors ( 96.05% vs. 89.55%; χ2=27.621 ) were significantly higher in girls than in boys ( all P<0.05 ). The awareness rates of adolescent physiological knowledge ( 63.72% vs. 41.58%; χ2=88.989 ), private parts ( 64.56% vs. 55.98%; χ2=13.926 ) and sexual harassment behaviors ( 93.80% vs. 91.17%; χ2=4.509 ) were significantly greater among grade 6 students than among grade 5 students ( all P<0.05 ), and the awareness rates of adolescent physiological knowledge ( 69.96% vs. 36.22%; χ2=207.041 ), adolescent health knowledge (58.99% vs. 33.44%; χ2=118.903 ) and sexual harassment behaviors ( 96.16% vs. 88.89%; χ2=34.754 ) were significantly higher among students living in urban areas than among students living in suburban areas ( all P<0.05 ). Primary school students mainly acquired sexual knowledge from their parents ( 1 208 students, 66.67% ) and teachers ( 1 161 students, 64.07%), and the source of life was the predominant demand for sexual knowledge ( 969 students, 53.48% ).@*Conclusions@#The awareness of adolescent sexual knowledge remains to be increased among grade 5 and 6 primary school students in Hangzhou City. More attention paid to male students and students living in suburban areas, and building a school-family-community educational network are recommended to increase the awareness of adolescent sexual knowledge among primary school students.

ABSTRACT

Objective To compare the changes of knowledge, attitude and practice (KAP) related to taeniasis and cysticercosis among primary school students before and after health education interventions in disease-elimination pilot areas of Henan Province, so as to evaluate the effectiveness of health education interventions. Methods A primary school was selected from each of Dushu, Bowang and Yangji townships, Fangcheng County, Henan Province from 2017 to 2019; then, 2 to 3 classes were randomly selected from grade 4 to 6 in each primary school, and finally, all students in these classes were enrolled as study subjects. A thematic health education lecture pertaining to taeniasis and cysticercosis was given once each year by means of “health education in class”. The KAP towards taeniasis and cysticercosis was investigated among primary school students using selffilled structured questionnaires, and the changes of awareness of taeniasis and cysticercosis control knowledge and percentage of healthy behaviors formation were compared in primary school students among years and following various frequencies of health education interventions. Results More than 10 thousand person-time primary school students received health education interventions during the period from 2017 to 2019, and a total of 1 223 person-time primary school students were investigated during the 3-year study period, including 633 person-time men and 590 person-time women and 88.55% of primary school students at ages of 10 to 12 years. The proportion of households raising pigs and using dry toiliets decreased from 5.30% (24/453) and 18.10% (82/453) in 2017 to 3.60% (13/361) and 11.08% (40/361) in 2019, respectively. The awareness rate of taeniasis and cysticercosis control knowledge increased significantly from 0.22% (1/453) in 2017 to 62.59% (256/409) in 2018 and 76.73% (277/361) in 2019 (χ2 = 567.60, P < 0.001), and the awareness rate was 85.25% (104/122) among primary school students attending the thematic special health education courses for successive three times. The awareness rates of taeniasis and cysticercosis control knowledge were 0.44% (1/225), 58.00% (116/200) and 71.63% (149/208) among male students and 0 (0/338), 66.99% (140/209) and 83.66% (128/153) among female students from 2017 to 2019, and there was a significant difference in the awareness of taeniasis and cysticercosis control knowledge between male and female students in 2019 (χ2 = 7.14, P = 0.01). On-site teaching by doctors was the students’ favorite way to receive health education interventions (70.07%, 857/1 223), followed by watching health education videos (58.63%, 717/1 223) and seeing parasite specimens (48.9%, 598/1 223). The proportion of students’ families who used cutting boards for raw and cooked food separately increased from 7.28% (33/453) in 2017 to 47.37% (171/361) in 2019, was 67.21% (82/122) among students attending the health education courses for successive three times. The proportion of students who frequently washed their hands before meals and after using toilet increased from 71.96% (324/453) in 2017 to 89.47% (319/361) in 2019 and was 95.90% (117/122) among students attending the health education courses for successive three times. In addition, the percentage of students washing hands frequently in families using dry toilets was significantly lower in those who did not use dry toilets (χ2 = 9.21, P = 0.002), and the proportion of students with a habit of eating raw or undercooked meat decreased significantly from 35.76% (162/453) in 2017 to 6.65% (24/361) in 2019 (χ2 = 69.11, P < 0.01). Conclusion The thematic health education activity of “health education in class” contributes greatly to the increase in the awareness of taeniasis and cysticercosis control knowledge and the rate of healthy behaviors formation among primary school students in diseaseelimination pilot areas of Henan Province.

ABSTRACT

Objective To investigate the effectiveness of a new health education pathway for echinococcosis control among primary school students in regions highly prevalent for echinococcosis in China. Methods Six primary schools were randomly selected from echinococcosis hyper-endemic regions, with 13 classes assigned to the intervention group and 9 to the control group, and all students in these 21 classes were recruited as the study subjects. Echinococcosis health education was performed through the pathway of assessing the current status-strengthening the building of teaching resources-focusing on practices in the intervention group, while routine health education was given in the control group. A questionnaire survey was performed to assess the score of echinococcosis control knowledge (including theoretical knowledge score and mean daily practical capability score) before and after the health education interventions to evaluate the effectiveness of this new health education pathway for echinococcosis control. Results The mean score of echinococcosis control knowledge was 68.86 ± 18.70 points at baseline, with the mean theoretical knowledge score of 40.97 ± 10.75 points, and the mean daily practical capability score of 27.89 ± 12.50 points. Clustering analysis showed three types of populations, including “unsatisfactory”, “learn and apply creatively”, and “rote learning”, which accounted for 24.62% (240/975), 45.74% (446/975) and 29.64% (289/975), respectively. The mean score of echinococcosis control knowledge was 81.08 ± 18.15 points in the intervention group during the final assessment, with the mean theoretical knowledge score of 43.65 ± 9.40 points, and the mean daily practical capability score of 37.43 ± 12.22 points, and both were significantly higher relative to baseline (t = −4.201 and −15.202, both P values < 0.01). The mean score of echinococcosis control knowledge was comparable between at baseline (70.55 ± 19.46 points) and final assessment (71.74 ± 19.37 points) in the control group (t = −0.87, P > 0.05). Conclusions The awareness of echinococcosis control knowledge is fair among primary school students in echinococcosis hyper-endemic regions; however, the capability of combining theoretical learning and practices requires to be improved. The health education mode based on the pathway of assessing the current status-strengthening the building of teaching resources-focusing on practices seems to remarkably improve the understanding of echinococcosis control knowledge among primary school students in echinococcosis hyper-endemic regions.

ABSTRACT

Objective To investigate the prevalence and risk factors of Blastocystis infections among primary school students in Jiangjin District, Chongqing City. Methods A cross-sectional questionnaire survey was conducted among students sampled from a primary school in Jiangjin District, Chongqing City on April, 2018, and their stool samples were collected for microscopic examinations, in vitro culture and PCR assays to analyze the prevalence of Blastocystis infections and subtype of the parasite. In addition, the risk factors of Blastocystis infections among primary school students were identified using univariate analysis and multivariate logistic regression analysis. Results A total of 466 primary students were surveyed, and the subjects had a mean age of (9.81±1.66) years and included 236 males (50.64%) and 230 females (49.36%). The prevalence of Blastocystis infections was 15.24% (71/466) among the study students, and there was no significance difference in the prevalence between male and fe- male students (16.52% vs. 13.91%; χ2 = 0.616, P = 0.433). In addition, there was a significant difference in the prevalence of Blastocystis infections among grade 1 (6.35%, 4/63), grade 2 (5.17%, 3/58), grade 3 (21.74%, 15/69), grade 4 (25.30%, 21/83), grade 5 (10.19%, 11/108) and grade 6 students (20.00%, 17/85) (χ2 = 15.410, P = 0.009). There were four Blastocystis subtypes characterized (ST1, ST3, ST6 and ST7), in which ST6 was the most common subtype (45.07%, 32/71), followed by ST3 (25.35%, 18/71). Multivariate logistic regression analysis revealed that minority ethnicity [odds ratio (OR) = 4.259, 95% confidential inter- val (CI) : (1.161, 15.621)] and low maternal education level (primary school and below) [OR = 9.038, 95% CI: (1.125, 72.642)] were identified as risk factors of Blastocystis infection among primary school students in Jiangjin District, Chongqing City. Conclusions There is a high prevalence of Blastocystis infections detected among primary school students in Jiangjin District, Chongqing City, and ST6 and ST3 are predominant subtypes. Minority ethnicity and low maternal education level (primary school and below) are risk factors for Blastocystis infections in primary school students.

ABSTRACT

Objective To understand the current situation and characteristics of potential rabies exposure to primary school students in the area with a high incidence of rabies of China, and to provide evidence for developing the intervention measures. Methods A population-based investigation was conducted in Guizhou and Guangdong provinces which had been the severe rabies epidemic areas in southern China. Two urban primary schools, two county schools, four township schools and four village schools were selected in the 2 provinces. Samples of the students were from each grade of the 12 schools for their potential rabies exposure information. Results Among 2408primary school students being interviewed, 290 person-times exposure were found during January of 2007 to May of 2008. The total exposure rate in the observed students in 2007 was 9.22%, but higher in Guizhou province (16.92%) The rate of exposure was higher in male students than in female, and was also higher in the students from rural than from the urban areas. More exposures were seen in summer season, from June to August. The kinds of animals that causing the exposures would include dogs (74.14%) and cats (25.86%). 32.07% of the total exposures were related to playing with animals. Arms and legs were the most affected areas of the body including, 37.59% of the wounds appeared below the knees. Conclusion The rate of potential rabies exposure was high in primary school students, especially in the rural areas. Playing with dogs and cats appeared to be the major behavior causing the exposures among the students.

ABSTRACT

Objective Trachoma is a preventable infective eye disease worldwide. The incidence rate of active trachoma in children can reflect the epidemical degree of trachoma in the location. This study tried to assess the prevalence and risk factors of trachoma in students at primary school in Wuqiang County, China. Methods An epidemiological survey was carried out in Wuqiang County in 2006. The investigators were from Wuqiang County Hospital and Beijing Tongren Eye Center and received special training. Primary school children aged 6- to 16-years-old were selected by a cluster sampling with the class shift as the sampling unit. Trachoma was clinically examined under the slim lamp and assessed based on the simplified classification for trachoma proposed by the World Health Organization (WHO). The C. Trachomatis inclusion body was detected from conjunctival swab of patients with trachoma using enzyme immunoassay and polymerase chain reaction (PCR). The questionnaire was performed in the examinees and parents. The survey followed the Declaration of Helsinki and written informed consent was obtained from the examinees. Results A total 379 cases with trachoma were determined in 1622 primary school examinees with the prevalence 23. 4% (95% CI:25. 5% -21. 3%) .including follicular trachoma in 81% and intense trachoma in 19% . The overall prevalence of trachoma was higher in female students than male ones (25. 8% versus 20. 9%, P = 0. 020). No inclusion body of the conjunctiva was observed. Sixty-four cases (38. 1%) presented the positive response for C. Trachomatis antigen by enzyme immunoassay, and 109 cases (64. 9%) were positive response by PCR. The multiple factors logistic regression analysis showed the risk factors for active trachoma included living location, suburbs and female and age(0R =0.763, 1.299, 1. 147 respectively). Conclusion Trachoma showes a disseminating procedure among primary school children in Wuqiang County. The prevention strategy should be made for related risk factors.

ABSTRACT

In order to assess the physical growth of primary school student in the rural area, body height, weight, chest circumference and sitting height of 923 students (469 males, 454 females) in the town of Angang, Kyungpook province, Korea, were measured from April to May, 1988. The most growth age of boy height was 10-11 years old in both sex and the growth curves were crossed at 9-11 years old. The most rapid growth age of body weight was 10-11 years old and growth curves were crossed at 9-11 years old. The most rapid growth age of chest circumference was 10-11 years old in female, whereupon female outgrows male by contrast with male's slow and linear growth. The most rapid growth age of sitting height was 10-11 years old and the growth curves were crossed at 9-10 years old. The Rother index was 1.17-1.30 in male, 1.16-1.29 in female, and male's index was in general superior to female's.
